Mafia II features an open world environment. The game's world is divided into three districts: Downtown, Midtown, and Uptown. The player can explore the world without any limitations, except for areas that are locked until the player progresses further into the story.

The player can drive a variety of vehicles, including cars, trucks, and buses. The player can also hijack vehicles. The game features an interactive soundtrack, with certain songs activated when the player enters certain vehicles.
